I give permission for my child to participate in the MHD Youth Talent Showcase and the audition process. I understand that participation in the final showcase on August 22 is subject to selection following the audition on August 15.

Media Release Consent

I also acknowledge and consent that, if selected, my child's performance at the MHD Youth Talent Showcase may be photographed, video recorded, live streamed, and/or otherwise captured for promotional purposes. I grant MHD permission to use, reproduce, publish, and share photographs, video recordings, and related content featuring my child on MHD's website, social media platforms, newsletters, and other community communications without compensation.
